East Valley School District has partnered with Yakima Valley Farm Workers Clinic to offer our families the opportunity to get their student's immunizations and sports physicals up-to-date for the upcoming school year.

The clinic will be held at East Valley High School on August 15th from 9:00 am - 4:00 pm. Immunizations will be available for children ages 3-19.

 

Appointments are required. Please call (509)864-1428 to schedule.

 

Parent consent and health history forms signed by parent/guardians are required. Forms will be provided on arrival. Please make sure to arrive 15 minutes prior to the appointment.

 

Please bring immunization card or record, if possible.
